Types of Vitrectomy

Pars plana vitrectomy (PPV) is the most common type of vitrectomy. A small incision is made in the lower border of the window, and the eye is carefully elevated so that the vitreous can be removed. The advantage of PPV is that it is a minimally invasive procedure and does not require stitches. PPV may be preferred for patients with thinned or transparentness in the posterior chamber, as well as for those who have poor vision in either eye.

Transconjunctival sutureless vitrectomy (TSV) involves making an incision just above the lower eyelid and then removing the vitreous through this opening. TSV has been shown to be more successful than PPV in achieving good vision due to the preservation of both the retina and choroid plexus. TSV also has a low complication rate, making it suitable for patients with high-risk factors for surgery such as severe dry eye syndrome or prior refractive surgery.

Endoscopic subretinal or epiretinal approach uses an endoscope to enter and remove the vitreous directly from within the eye without making any external incisions.
