Types of Corneal Transplant Surgeries

There are a variety of types of corneal transplant surgeries. These include penetrating keratoplasty (PK), descemet’s stripping endothelial keratoplasty (DSEK/ DSAEK), descemet’s membrane endothelial keratoplasty (DMEK), deep anterior lamellar keratoplasty (DALK), and superficial anterior lamellar keratoplasty (SALK). 

Superficial epithelium is removed from the donor cornea using a blade, and then the thick Descemet’s Membrane is replaced. In DSEK, the Descemet’s Membrane is first cut into smaller pieces, and then these pieces are sewn back onto the donor cornea. This surgery is typically done on an outpatient basis, but may require overnight observation in some cases. DMEK replaces only the Descemet’s Membrane, without removing the underlying epithelium. This procedure can be done as an outpatient or under local anesthesia with minimal postoperative pain. DALK repairs a tear in the stroma of the eye caused by an injury or previous eye surgery. The surgeon removes a small piece of extra-ocular tissue called a flap, which is grafted onto the area of damage in order to help restore stability to the eye and prevent future vision loss. SALK is a newer type of surgery that uses a laser to remove superficial epithelium and the innermost layer of the stroma, leaving Descemet’s Membrane intact. This surgery can be done as an outpatient or under local anesthesia with minimal postoperative pain.
